Please read the recommended referral process before making a referral. Note that a Service Agreement with the Support at Home funder must be in place before a referral is made.

Introducing your Occupational Therapist

Mark Graham

Mark has 20 years of experience in Community Based and Hospital services in Western Australia, Canada and Queensland.

He has specialized in Aged Care for most of his career, and highlights have included working in the Advanced Care of the Elderly Program at Townsville Hospital and the Integrated Stroke Program at the WA Country Health service. More recently, Mark has worked for the Queensland Department of Housing, a Home Care Package provider in Western Australia, and for the Transition Care Program based at Mareeba Hospital.

Mark is also an experienced Mental Health Clinician and Music Assisted Therapist.
Mark is highly skilled and trained in the assessment and management of dementia, falls / syncope, stroke, and neurological conditions.

Mark can prepare plans for pressure injury prevention and management, as well as falls prevention and the management dependent oedema. He has recently completed training in lymphoedema management.
Mark is excited to be bringing this wealth of experience to his local community on the Atherton Tablelands.

Mark enjoys bushwalking, gardening and amateur theatre.
